Appointments

Appointments are necessary for any treatment. We trust that when you make an appointment with us that you will make all the necessary arrangement to be here. Our schedule is by appointment only, so that we can ensure each patient a timely visit. We ask that, in return, you are prompt to your appointments and follow our strict 48 hour cancellation policy. A charge of $80.00 for missed hygiene appointments, $250 for missed appointments for Dr. Kelly or appointments that are not cancelled or rescheduled within the 48 business hour required notice to the office.

Forms of Payment

Our office accepts several forms of payment for dental treatment. We accept Cash, Personal Checks, Business Checks (by an authorized person), MasterCard, Visa and Discover. There is a Service processing charge of 3% if you are utilizing any credit card processing to pay for your services. 

We also offer CareCredit patient payment plans. CareCredit is a revolving charge account that is easy to use, takes just minutes to apply, in most cases is interest free, and provides several payment options with no upfront costs and no annual fees. CareCredit can be reached at 1-880-839-9078 or www.carecredit.com .

We offer "Smile Club" for patient with no insurance, please contact the office for additional information.

Dental Insurance

Our office is a participating provider with all CareFirst (No HMO plans), Cigna PPO (No HMO's) & GEHA (No HMO's).

We also are a non-participating provider for many other insurance carriers.
Patients with coverage from these carriers will be required to make full payment to our office at the time of their appointment. Reimbursed payments (per non-participating agreements from their insurance carrier) will be sent to the patient at their home address.

As a courtesy to our patients, our office will submit claims for the patient’s insurance carrier.

Emergencies/After Hours
For patients of record only – you need to leave a message at the office, and someone will return your call within the next business day.
If you have severe problems or pain, go to the emergency room or walk-in clinic directly if this occurs during non-office hours.
If the doctor is out of town, the current doctor covering will be contacted by our staff for you.

As a licensed Dentist in Salisbury, MD we work with our patients so they can achieve and maintain a beautiful smile and healthier gums and teeth.
