We are accepting New Patients, but we are working off of a waitlist and cancellation basis of when you would be called to be scheduled. If you have a pain or problem give us a call to see what is available for you to have a Emergency Exam and X-ray of the Pain/Problem area.

We are currently booked 6 months out for Hygiene Appointments. When you become a patient with us your routine appointments will be pre-scheduled for you at each visit.

If you would like to be added to our New Patient List, follow the directions below on what must be sent to us via email, fax or physical drop off before becoming a new patient with us.

New Patient Information
Before you could be seen in the office or schedule an appointment, you must complete these steps:
Step 1: Fill Out the Medical History Form (Link Below)
Step 2: Fill Out the Dental History Form (Link Below)
Step 3: Please provide a list of ALL current medications that you are taking
Step 4: Email a photo of your Photo ID
Step 5: Email a photo of your Dental Insurance Card (Front & Back) 
Step 6: Contact any of your previous Dentist(s) for your Xray's & Clinical Notes.

Are you are on a blood thinner,  take an aspirin or in need of a pre-medication?
Please list the Doctor who prescribed those to you &/or what surgery Doctor you had that prescribed a pre-medication. We will need to send a Medical Clearance to your prescribing Doctor's, so we ask that you please have their information listed on the paperwork for the front desk to send via fax or email for you.

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. Educate yourself on your symptoms by reviewing the content on this Website. Also, take some time to review our staff page and familiarize yourself with the doctor. We look forward to seeing you & your smile at your first visit!

Driving Directions 

The address we are located at is 1820 Sweetbay Drive Suite 104 Salisbury, MD 21804. While driving on Sweetbay Drive you will see the Fountains on your right hand side, you will come to a cul-de-sac and we are the building on the right-hand side, then continue to drive towards the straight towards the dumpster and we are on the end on the right. Our office is located at SUITE 104 you will see a teal-colored sign.
